When I get all the way through to the PDF via FOP, the only entity that is
translated is the ampersand in the second cell, i.e.:
&#38;ITEM
&ITEM
&lt;ITEM
There is something wrong with your workflow or whatever you name "all 
the way through to the PDF via FOP". I just tried such a trivial stuff as
<fo:block>&#38;ITEM &amp;ITEM &lt;ITEM	</fo:block>
and got as expected
&ITEM &ITEM <ITEM
