I believe when bookmars were introduced it didn't matter where they were declared in the FO document. With 0.20.1 or so streaming rendering was introduced in order to reduce memory consumption, and since then it matters. Bookmarks are regarded a "root" extension and will be 'rendered' when the end tag of the first page sequence is encountered. This means bookmarks must be completely declared *before* the first page sequence, or they'll just be dropped. I'm sure this is an unintended consequence, but one which bothers me *much* less than others.
