Looks like you forgot to declare the XSL-FO namespace.

The top-level element in your XSLT must look approximately like this, if
you use the often-used "fo" namespace prefix:
<xsl:stylesheet version="1.1" xmlns:xsl="http://www.w3.org/1999/XSL/Transform"; 
xmlns:fo="http://www.w3.org/1999/XSL/Format";>

If that's not the problem there must be a problem with namespaces in the
XML parser you're using.
