Hello Mirko, you may use upCast to convert your MS Word file to XML (please refer to www.infinity-loop.de/products/upcast/). Compared to other tools on this field, upCast has proved to be a very mature and solid solution for tasks like these. We use this tool a lot and are very satisfied with it. Anyway, upcast will convert your Word document into a proprietary XML format which tries to match typical RTF constructs to XML tags. That means, before you can enjoy some pretty xsl:fo output you have to do some XSLT programming to transform upCast output data to FO (maybe someone has done this before; perhaps I'll do it myself when there is some time left). As far as layout is concerned, upCast uses CSS to handle stuff like fonts, page settings and the like. Since fo syntax and css have a lot in common, it should be easy to exploit the CSS information to reproduce the layout of your Word document.
We are transforming the data from upCast into DocBook and generate FO output afterwards - thus, I have no direct remedy to offer so far. Hope this helps you a bit.
