Sounds like you have something like
  <xsl:template match="parent-of-employer-and-general-and-employees">
     <fo:flow>
         <xsl:apply-templates>
     </fo:flow>
  </xsl:template>

  <xsl:template match="employee">
    ...

You probably want the first template to be
  <xsl:template match="parent-of-employer-and-general-and-employees">
     <fo:flow>
         <xsl:apply-templates select="employee">
                              ^^^^^^^^^^^^^^^^^^
          ...

why this is so.

the default output encoding:
