'xsl-region-body' does not map to the region-body in page-master 'blank'
That's a well known bug in one of the recent DocBook XSLT releases.
I vaguely remember there is a fix at least in the DocBook XSLT CVS
repository, or perhaps even already a new release.
Other than than, you can search the style sheets for the page master
named blank and remove the flow-name from its fo:region-body.
