I am not sure, but I think it should be possible to drop one of the commercial implementations into Cocoon. Other than that, the best solution would seem to be to contribute to the open-source work. Neither of the issues that you mention is easy to solve, and it is likely to be some time before an open-source solution is available. If you need a solution today, I think you have to look at the commercial implementations. BTW, in my previous posting I used the word "bidi" where I meant writing-mode. The answer is the same.
