To make the intermediate FO file do this in the fop-0.20.5/ directory:
WINDOWS: xalan.bat -IN file.xml -XSL file.xsl -OUT file.fo
UNIX: xalan.sh -IN file.xml -XSL file.xsl -OUT file.fo
file.fo will have the information we need to see for ourselves. Hopefully it'll be a brief/simple document which demonstrates the problem.
