I only see hourly, daily, and weekly upstream as well... looks like that is
a documentation error. I filed a bug here:
http://projects.theforeman.org/issues/18394

If you want to request a montly sync plan, file a feature request here:
http://projects.theforeman.org/projects/katello/issues/new

To create a montly sync plan now you could use a cron job with hammer as a
hacky workaround :)
