the requirements are:

1. The project is licensed under a license approved by the Open Source 
Initiative.
2. The project source code is available for download.
3. Your open source project has a publicly accessible website.
4. Atlassian's software is accessible to the public.

we have all that.

did you pick the wrong form?

many open-source projects are not official non-profits, so it does not
make any sense for them to ask for that.
