I found a tip on this forum that worked for my problem: Replacing backwards
This hardens my assumption that PSPad gets out of sync with the text, as the text ahead (=still to process) is changing (apostroph = 1 byte, new line = 2 bytes, therefore shifting 1 byte) and PSPad tries to replace at the old locations, but these don't fit anymore and nothing happens. When replacing backwards the text "ahead" (=still to process) doesn't change and all found locations don't shift, hence PSPad can replace them.
