I'm not sure I agree with this - very often when working on a website project I'll have multiple types of file (.php, .html, .js, .css et al.) open, and when I need to do a multifile S/R it will generally be on one type of file (HTML, usually).
If I ever did need to ignore the extension it would be unusual behavior and I would expect to have to edit the file extensions textbox. If any changes are to be made in this area, I'd much rather see a checkbox list of extensions of the currently open files. Possibly a "..." button next to the textbox which opens a dialog containing said checkboxes; the button being visible only when 'Open Files' is selected.
