This should match rev 2d closely…  The only change in 3a is that I converted the source from MS Word 2003 to FrameMaker 7.1.  Editing the word doc has generated fatal errors in creating the PDF.  I believe that a contributing factor is the age of the doc.  This doc was first imported into word in 1994.  Since that time it has been updated using Word 95, Word 98, Word 2000, possibly Word XP, and finally Word 2003.  I did an analysis of the word doc by outputting to XML and found that there was a lot of unnecessary material that had been carried forward for a long period of time.  As a result, the document has become very hard to manipulate.

 

I will be posting FrameMaker sources for archiving on the FTP site.  The PDF is about the same size as published using word.  The bookmarks have improved and all the links are hot once again.

 

This conversion required that all the links be remade by hand.  Please look the doc over and make sure that links are correct and table formatting is consistent.  I was able to bring all the words across, so the actual text should not be an issue.  I did remove several hanging indents during the conversion process, so there will be differences in some of the heading numbers.
