Hi, We were these kind of issues with the PoE modules which we loaded on to the SX1600. Try with different slots as suggested by Jared. It might be a problem with the line card. We replaced nearly 10nos. of line card(PoE) due to firmware issues with SX series which was reporting false indications.
I think this is quite common. Can you paste your "show chassis" or "show system" command output also "show modules" if it supported.
