Maybe this is a crazy thought, but have you checked the style of that heading to make sure it isn't set to be white (or whatever color your background is)? That would sure cause the results you're describing.

When I generate the TOC, all of the Heading2 level entries are invisible; text, dots, and page numbers. They still take up space in the table as if they were there and the blank space hypertexts to the correct place in the manual. I know what caused this. I copied a book from a different project, including the TOC and IX files. Renamed them, then generated new TOC and IX files for the new book. Then I deleted the autogenerated ones and renamed the old files as the new ones. This way I kept all of the formatting from the old project. The problem is that I only wanted Chapter and Heading1 in the first book and added Heading2 to the second. Is there a place in the TOC section or on the Reference Pages to make the Heading2 visible? There is a Heading2 entry on the TOC reference page. It looks normal. I already checked the paragraph format for Heading2TOC. It's fine and there are no character formats applied. The only thing I can think of is to nuke them, regenerate the CORRECT way, then reapply all of my formatting manually, or through format copy. I'd rather turn on something that is missing than go through all that.
