In your pgf tag definition, on the Advanced tab, select a Frame Above Pgf or Frame Below Pgf. You can use one of the default choices or create your own on a reference page (that's where the defaults reside). Play around with the height of the frame containing your horizontal rule, and the position of the rule within the frame, to change the space above/below your Q&A pairs.
