First the rule may be like that for Hart's Rules for Writers, but if
you use Chicago, MS, or most other style guides, particularly those
for electronic publishing, I believe (no, I haven't checked them all
before replying ;- ) ) they'll use your previous convention.
Then, the numbering....
There isn't a compelling reason to use or not use <$chapnum> for
appendix numbering.
To use it, in the book file, you'd highlight the first Ap, right click
to bring up its numbering properties, and set the Chapter property to
force A. Do the same thing with subsequent Aps but set the property to
increment based on the previous chapter.
You can get the same effect if you numbered based on a string in your
ApTitle tag that has something like: A:<n=A> for the initial Ap and
A:<n=A+> for subsequent Aps.

To resolve this I have numbered my styles as follows:
Appendix x:Appendixes
AppendixTitle x:Appendix <$chapnum>
"Appendix" must be the first paragraph in the first Appendix file.
Thereafter it is not used in subsequent Appendix files, only
"AppendixTitle".
This is a bit klunky bit it gets the desired result in the TOC output.
Is there a more elegant way to get the results I am after that my tiny
mind can't think of?

<$chapnum> is a document-wide counter; it cannot be incremented within a
document.
It can be set locally within a document, but it is typically controlled
through the book numbering settings.
If <$chapnum> is to be used, each chapter must be a separate FrameMaker
document (or it could be split into multiple documents, where <$chapnum>
is
set not to increment in subsequent chapter parts, through the book
numbering).
