Yes, there is 'FormCreator' an add-on for FrameMaker. There are FrameMaker files with many text and some checkboxes ... radio buttons ... signature fields the add-on add field information at the paragraph formats (or at paragraph formats in the table) and the output is an fillable PDF form.
