I've managed to add the page manually and have it regenerate properly -- though this only works when I know I've got an odd-paged TOC.
I add two paragraphs at the end of the TOC -- one that bumps to the top of the page, and a second that has my "Intentionally left blank" text (about 250 points down the page). Then regenerate. This usually takes -- I have occasionally had to do it twice (every so it moves to the top of the TOC -- not pretty!)
