It's the format example for a dictionary-style header. 

Ordinary running headers take the form <$paratext[paratag1,paratag2,
...]>, which tells FM to display the text of the first instance on the
page of one of the listed pgf tags. For instance,
<$paratext[Head1,Head2]> would display the text of the first Head1 or
Head2 pgf on the page. 

Prepending the plus sign tells FM to display the _last_ instance on the
page. For instance, <$paratext[+,GlossaryHead]> would display the text
of the last GlossaryHead pgf on the page.
