I've "managed" to do this a little differently than the others have mentioned. We start the content that needs the change bar prior to the graphic, include the graphic, and then at least the first character of the para immediately follows. Yes, it then looks like there is a change in the first line of the next para, but seems to work well enough for our people. Not necessarily the best way, but it works.
