Thanks Art and Stuart, I needed to turn on the 'Create Named Destinations for All Paragraphs' option. I have a feeling this issue has come up before, but I'd forgotten about it!
BTW - I'm printing a postscript file and then distilling, because it seems to be the only way to get the book into a single PDF file. If I've missed something, please let me know.
