I still don't know why, but when I removed the "book-within-a-book" it all worked fine. I shall investigate further, but have Plans B to Z in case I can't find a proper solution. I now need a lie-down in a darkened room, especially as the country will grind to a halt tomorrow because of some wedding or other. More beer, I think.
