That's more or less what I did. I do have the PDF file and there is no index.
My apologies. You're right. There is no true index in the PDF. I was thinking of the bookmark index that displays on the left-hand side of the page. Of course, you can search a PDF, which sometimes gives better results than a poorly done true index.
