Could you describe how you're working when editing or regenerating the index? As Richard described, setting the format on the reference pages and then update these formats should stick after regenerating the index. Note that's update the paragraph style, not the character style! It sounds like you don't reuse the once generated index, do you? MasterPages can be applied automatically by using either a dedicated Index template file or the Apply MasterPages functionality. If you don't want to regenerate the index because of the many manual adaptions you have to make, consider the option to use a script to do that. It will save you a lot of time! Also setting the right options in your paragraph styles can help. Either way, finalizing the index using manual tweaks should only be done when the content of your book is also finalized - don't do that for in house reviews or whatever ...;) Note: you can set up these settings (styles, masterpages) in your default template that also be used by the regular chapters. Then these will be applied even when you generate a new index in a new book.
