Are you still using spaces to push the RH parts around? Use a right-tab with 
both left and right parts of the RH in one frame. Like

Left part of RH --> right part of RH

If the tab is a right tab (not left), the right part of the RH will extend from 
the right side of the frame back to the left side of the frame. This will only 
become a problem if the topic heading is longer than the remaining space in the 
frame.

The other limitation of this system is formatting. If you want your RH to 
display one or two italic words (with the rest roman), you have to improvise 
something different (like a different master page).
