I am having the same problem with FM10. My Acrobat version is 8.1. I go to create the PDF, set up which paragraphs I want as bookmarks, and create the PDF with the appropriate bookmarks in the file. I go back to the book later to recreate the PDF and my bookmark settings are lost and I have to recreate them. It's painful, given the awfulness of the PDF settings dialog (come on Adobe, surely you can at least give us multi-select!).
It doesn't happen with all my books, but does with most of them. I haven't figured out a pattern yet.
