Very basic questions:

Has it ever worked correctly for you? Or did you have this problem when you
got it?

Have you examined the mechanism with the cover off? Does anything seem
amiss? E.g. if the film is pooling instead of spooling that suggests the
take up hub isn't turning or lacks torque... in which case it would turn if
there's no spool on it, or just a spool but not pulling film. Is the
mechanism loosing either the upper or lower loop?

Since it can be hard ti visualize problems from written descriptions, can
you shoot a short video of the problem occuring with your phone upload it
to Youtube or some other video host site, and post a link here (and/or
other sites with 16mm knowledge) so we could see what you're talking about.

(Hopefully you have some slug to run tests, or can get some from some kind
maker in your area.)

Caveat: There are some 16mm cameras a mechanically inclined amateur can
take apart and futz with some. IIRC a Bolex isn't really one of them...
