There have maybe been more than one thinks, particularly if we include independent narrative films in addition to avant grade work. I believe Lukas Moodysson's CONTAINER was exhibited on 16mm. I suppose Kelly Reichardt's MEEK'S CUTOFF was released digitally even though it was shot on standard 16mm, but I'm not certain.
