I may or may not have hit some variation of this bug, I'm not sure.

My system is on 14.3 and has a Qlogic QLE2560, with an HP LTO4 drive attached.
Until today, everything was working fine.
Today, I rebooted. I had some uptime, but I don't remember not rebooting after
the last "freebsd-update". So theoretically, nothing should have changed.

But today, I rebooted for unrelated reasons. And I got caught in a boot loop
with a kernel panic emanating from "firmware_taskq".
Googling around a bit, I stumbled upon this issue and related reports about
Qlogic on the mailing list.
I pulled the Qlogic card, and the system resumed booting fine.
I swapped PCIe slots to no avail, hoping in vain it might be some kind of
electrical issue on the board (Supermicro X9SRi-F-B).

But then I happened upon a crazy observation: Enabling the Qlogic's "OptionROM"
made the issue go away!
I'm not sure what is going on there, but I left the card out of the system
after that observation.
The system is in a remote location, and I don't feel comfortable any more with
the card in there. Will take the card home with me, so I can experiment on the
issue some more.
