It is present in 12 as far as I can tell:
# find /usr/include -name stddef.h -print | more
# uname -apKU
FreeBSD FreeBSDx64OPC 12.0-CURRENT FreeBSD 12.0-CURRENT r325700M amd64 amd64

My guess is something is odd with your environment.
