I think I've seen this sort of thing being caused by incorrect modifier key state - that is, it's acting as if ctrl and/or alt are being pressed. If you get in this state again try pressing and releasing modifier keys to see if that clears it up. If so, it's a bit of an unfortunate workaround but at least will provide a hint for next debugging steps.
