People with many different networks using real IPs generally don't need natd, they simply use a router and/or firewall.

This being said, you can use natd with real IPs exactly the same way as you would for RFC-1918 unroutable ones. You can run natd multiple times by incrementing the divert socket # for each and have each natd talk to a different divert socket.
