I conducted another experiment:
cp /bin/ls /usr/lib/ 
apply freebsd-update fetch install

And freebsd-update did replace the bogus /usr/lib/
So I can conclude that it WILL replace modified files (unless they are
listed in UpdateIfUnmodified).
