What does it take to reliably change to an alternate keymap immediately? kbdmap and variations of kbdcontrol -l aren't making any difference, although somehow yesterday they did. At one point, anyway.

Using Bulgarian BDS (bg.bds.ctrlcaps.kbd) as a test, "a" and "q" reversed positions, shift-zero became "=", and so on.

But I can't replicate that now. The Handbook L10N section on console setup says that kbdmap should work:
