I've had similar problems with 11.3 and 12.0 (unsure of patchlevel) when adding 
users using Ansible. Had to add a step in our script that runs pwd_mkdb after 
each user is added to make sure it is reliable.

I've had varied results with the same thing from 11.2 to 12.1.
Sometimes it works, oftentimes it does not. pwd_mkdb does fix it
up, but would be nice to not have to do that.
