Details:

Patch #3548 removed some hardcoded combat bonuses. This patch rewords the
combat section a little for this, although it's still heavily tied to the
classic ruleset (but now acknowledges this).

This text is very old. I haven't checked thoroughly that it's all still true.

Also:
* Merge "Modifying Combat Rules" into main section
** killstack is now a server option
** just note the possibility of tired_attack rather than explaining how to
edit rulesets
* Acknowledge existence of special "bombard" attack (but not really adequately
describing it)
