Details:
Add new create_frame() function for sdl2-client to be used where putframe()
has been used in sdl-client but cannot be used in sdl2-client. Initial commit
also makes icon2 widgets to use it (most notably this fixes selection of the
style for the nation - now one can see which one is currently selected).
