there is some "interesting" stuff in the output of your system :)

Does FDISK really only show the drives up to Y: while the kernel "sees" drive 
Z:?

Have you copied the kernel messages manually? It shows "P:[...] CJA 3144-1-1"? 
Should be CHS instead of CJA. And also, is this really for P:? According to 
FDISK, P: starts at virtual cylinder 3406:

> P:                 2055  FAT-32 LBA         1%  3406/001/01   3667/254/63

The following two kernel lines say "[...] with calculated value 3667-254-63". 
That in fact is what FDISK also detected as the end of P:

> Warning: using suspect partition Ext:13
> FS 0c: with calculated value 3667-254-63 instead of 596-254-63
