Now there's a fork of VSBHDA that "adds support for using soundfonts for
MPU-401 emulation using TinySoundFont, and properly fixes it to make it
work with games like Duke Nukem 3D." But it requires a lot of memory; from
the website: "Requirements remain the exact same as upstream VSBHDA, except
it now requires potentially 128MB of XMS memory for soundfont
functionality."

Soundfonts can vary A LOT in size. SoundBlaster AWE32
sound cards had between 0 and 28 MB of RAM for user
data and a pre-installed 1 MB font. Typical RAM sizes
were 1/2 MB, 2 MB, 8 MB or 28 MB (actually 32 MB of
which you could only use 28 MB due to the design).

I vaguely remember having upgraded mine to 2 or 8 MB
and already having reasonable fonts for that size :-)

Some common fonts used by the Linux fluidsynth softsynth
are between 3 and 150 MB in size, though.

I guess it would be nice if somebody could recommend
a small font with good quality to Cacodemon345, so
DOS users with less RAM could use that font then :-)
