Scott, if you are authenticating out of a passwd file, the mailbox for the users should be created automatically in /var/spool/mail/<userid> (assuming that's how you have your MTA configured). As far as an accounting system, you will have to develop one yourself or purchase a commercial one. I don't know of any off the top of my head, except for large enterprise Oracle based systems like Portal (which can cost millions of dollars).
If you are getting radius to authenticate out of a database, you can make some small modifications to icradius' radius.cgi to make it work with FreeRadius. As far as having it create mailboxes, you will have to get your MTA to authenticate out of a database aswell.
