Sorry I cannot answer your question Jason, this is a
"me too" too !

At the moment I only have a DEFAULT + DEFAULT in the
usergroup table and I can auth OK but when I add an entry
in the usergroup table as UserName=test GroupName=DEFAULT
then that test user no longer gets authenticated. I should
say that in using "radtest" it works Ok (from a different
test box pretending to be a "NAS") but not from this other
RADIUS (Radiator) server proxying to this box... I see this
in the logs...

 rlm_sql: Pairs do not match [test]
   modcall[authorize]: module "sql" returns notfound
 modcall: group authorize returns notfound
 auth: No Auth-Type configuration for the request, rejecting the user
 auth: Failed to validate the user.
 Login incorrect: [test/ \350\255\036\017[\313>\310nbK\212v:]\
  (from nas lei port 6186 cli 755269205)
  WARNING: Unprintable characters in the password. ? \
  Double-check the shared secret on the server and the NAS!

The other end (Radiator) absolutely swear they have triple
checked the shared secret so I'm dubious about the error
being reported here. This same setup works in other situations
with the same user/pw data. I wish I could supply better
diagnostics but I'm grasping at straws and understanding
how DEFAULT group entries work in SQL would surely help.
