To be blunt: administrator misconfiguration. The shared library loader has to be told where the shared libraries exist. There have been a number of messages on the list from people who put the MySQL (or other) libraries somewhere non-standard on their system. When the server won't start, they blame FreeRADIUS. Personally, I blame libtool, too. It's cross platform, it's nice, and it throws away all of the helpful error messages that make an administrators life easy.
