I have Simultaneous logins working with SQL, but we have some customers that we are providing authentication for who have multiple NASs that a user can be connected to.
Does freeradius support running radcheck on all the NASs to see if a user is logged in, not just the one he is coming from currently. - List info/subscribe/unsubscribe? See http://www.freeradius.org/list/users.html
