The only custom built module I tested with live traffic is appending a
line to a file. I did a stresstest with radclient it easily ate about 2700
requests/sec.

As I stated, I fixed it by disabling it. I enabled rlm_details to make
it "out of the box" with no custom code.

I have not seen any of these problems benchmarking with radclient.
I'm sending the same start-, alive- and stoppacket over and over, however.

My problem may already be fixed, I just pulled the latest CVS version (and
fixed some minor typo in a checked-in sql_postgresql..., and another
typo(?) in rlm_unix) and compiled it and updated all our servers. I'm not
seeing any problems right now, but maybe they come in a couple of hours
when the traffic will get heavier (peaking at 50-60 requests/sec).

Maybe I bothered you guys unnecessarily, sorry ;) I should've pulled the
CVS for starters..
