Unknown attribute SQL-User-Name
  modcall[authorize]: module "sql" returns fail

Anyone could tell me why I not seeing a SQL Query on
the screen?  Instead I get Unknow attribute SQL-User-
Name

Does your dictionary file include an entry for SQL-User-Name?
