At a guess, your accouting packets do not contain a NAS-Port attribute.
Possibly your RADIUS client is sending a usable value in NAS-Port-Id,
or somewhere else. (Dunno much about VoIP :-(

If you're not using the radutmp file, then the easiest thing would be
to disable radutmp in your configuration.
