I just wondering best way to get really sensitive comms, so a 5W HT could go for miles and miles.

The Wenet project shows what can be done with careful engineering of the modem, e.g. sending HD images at 100 kbit/s @ 50mW over 100km:

The key is the demodulator. For your application I'd use the FM HT to Tx, and a $20 SDR with a LNA as the Rx.

Written by a good friend of mine, Bill, VK5DSP. Try plugging numbers into this to get the range you want and understand the trade offs.
