I hope that you find the following suggestion helpful. When trying to
fix problems such as these, pay attention to the URL. The URL will tell
you what actions and screens are involved. Then go view the source for
those actions and screens. Pay particular attention to any files that
those actions or screens "include". You should view those files as
well. Also (I have seen this a lack of this in other posts), when you
want to add something to a screen or action, try to see if you can steal
some code from the other actions and screens. These suggestions may
sound basic, but we all need to be reminded of the basics once in a
while.
