Summary: IVTV Support for driver 0.8.0 and higher

Initial Comment:
Under kernel 2.6.18 and higher the ivtv driver must bu
0.8.0 and higher. With this driver a lot of the old
ivtv ioctl calls have changed.

The current implementation of ivtv.py does nothing when
the driver version is higher than 0x800, which means
that codec settings in local_conf.py are ignored.

This is not a major problem because the default setting
work correctly.

The new ioctls do need to be implemented at some time.

The changes to v4l2 ioctls are quite substantial, but must be done, I hope
to have a working version in the next couple of days.
